Certificate of identification
  • fees

(1) The Oregon Health Licensing Agency may issue a practitioner a certificate of identification to practice outside of or away from a licensed facility.

(2) The Board of Cosmetology shall determine requirements and the agency shall establish fees for issuance of a certificate of identification.

(3) Practitioners performing services outside of a licensed facility under the provisions of a certificate of identification shall comply with the safety and infection control requirements under ORS 690.165 (Powers of board) and 690.205 (Rules) and rules adopted thereunder.
